Aurora, CO 80014: On or before 9/23/21 Supreme Service LLC as the creating and exposing employer did not protect employees from injury in that roofing employees were not given effective information and training on the fall hazard associated with the work being performed as well as how to effectively use their assigned personal fall arrest systems. Employees were working with their lifelines maxed out. Anchors were connected to multiple lifelines. These conditions exposed employees to fall hazards of 8 feet and 10 feet. An employee working at the edge of the roof with an 8 foot fall hazard had his lifeline fully extended and would not have prevented a fall.
