29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) Beyond Roofing LLC, Inc. at : On November 4, 2020, the exposing employer did not ensure that employees utilizing extension ladders to access the first level roof on a two story residential home had the ladder extended at least 3 feet above the upper landing surface. One ladder was placed on top of a juniper bush. The second ladder was not secured and the side rails did not extend at least 3 feet about the upper landing surface. At least two roofers were utilizing the ladders for access while carrying materials up the extension ladders preparing for installation of the shingles and other materials. This condition exposed employees to falls from height of at least 10 feet. |

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): (a) Beyond Roofing LLC, Inc at 335 Carina Circle in Loveland, CO 80537: On November 2, 2020 and November 4, 2020, the exposing employer did not ensure that employees engaged in re-roofing of a two story residential home were protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Four roofers were working at levels between 15 and 25 feet removing and preparing to install roofing shingles without the use of fall protection. This condition exposed employees to falls up to 25 feet. Beyond Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1329255, citation number 1, item number 1, and was affirmed as a final order on 08/17/2018, with respect to a workplace located at 5726 Lisbon Way in Centennial, CO 80015. |
