29 CFR 1926.451(a)(6): Scaffolds were not constructed in accordance with the design by a qualified person: a) Ranki Contruction, LLC at 68 Eagle Ridge Drive Granby, CO 80446: On or before May 10, 2018 the exposing employer did not ensure that a wooden box used to move personnel was designed by a qualified person. A wooden box that was designed to move waste, was used to move personnel from the roof of a two-story single family home. The box tipped over, and the two employees fell approximately 15 feet. This condition resulted in exposure to fall hazards of approximately 20 feet. |

29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: a) Ranki Contruction, LLC at 68 Eagle Ridge Drive Granby, CO 80446: On or before May 10, 2018 the exposing employer did not ensure that a platform used by a rough terrain forklift to move personnel from the roof of a two story house was properly attached to the forks. A wooden waste box was used to move personnel from approximately 20 feet off the ground. The box tipped over with two employees in the box, resulting in these employees falling approximately 15 feet. This condition exposed employees to fall hazards of approximately 20 feet. |
