OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards: (a) Nebco Services LLC at Nitzel #1, County Rd 4: On and before September 7, 2012 the employer did not ensure employees were protected from struck-by hazards in that the wire rope used by the cathead on the off-driller side, to transfer drill pipe and equipment from the catwalk to the rig floor, was heavily damaged with obvious breaks and evidence of crushing. Employees working on the rig floor were exposed to struck-by hazards associated with failure of the wire rope. |

29 CFR 1910.151(b): There was neither an infirmary, clinic, or hospital used for the treatment of all injured employees in near proximity to the workplace nor a person or persons adequately trained to render first aid: (a) Nebco Services LLC at Nitzel #1, County Rd 4: On and before September 7, 2012 the exposing employer (drilling contractor) did not ensure that employees working in remote locations had access to medical attention, in that the closest medical facility from the drilling rig was located approximately 3 miles away at Sedgwick County Memorial Hospital. Employees were not trained in rendering first aid. This condition exposed employees to health hazards due to delayed first aid treatment. |

29 CFR 1910.184(d): The employer did not ensure that each day before use, slings and all fastenings and attachments were inspected for damage or defects by a competent person designated by the employer. Damaged or defective slings were not immediately removed from service: (a) Nebco Services LLC at Nitzel #1, County Rd 4: On and before September 7, 2012 the exposing employer (drilling contractor) did not ensure that damaged slings were immediately removed from service, in that employees attached frayed and damaged wire rope slings to hoisting equipment to lift and move equipment up to the rig floor. This condition exposed employees working on the rig floor to struck-by hazards as a result of the wire rope slings failing. |
