29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: (a) Highpointe, Inc., 6911 W 3rd Street, Greeley, CO 80634: On and before June 9, 2016, the controlling employer did not ensure that subcontractor employees were protected from struck-by hazards. Framer subcontractor employees were framing and setting walls on the second floor without the use of hard hats. This condition exposed employees to struck-by hazards. |

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems unless another provision in paragraph (b) of this section provides for an alternative fall protection measure: (a) Highpointe, Inc. at 6911 W 3rd Street, Greeley, CO 80634: On and before June 9, 2016, the controlling employer did not ensure that each employee working on the second floor was protected from fall hazards. Framer subcontractor employees working on the second floor were cutting studs, and were using pneumatic nail guns to form and frame walls, and were raising and setting the walls, without the use of fall protection. This condition exposed employees to a fall hazard of approximately 11 feet. |
