OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to explosion and burn hazards. (a) Golden Aluminum, Inc., 1405 E 14th St., Ft. Lupton, CO 80621; On November 10, 2021, the employer did not ensure employees were protected from explosion and burn hazards. An employee working in the Caster area sustained burns when the molten metal being released from the Selee filter bowls exploded upon making contact with a sow mold that had not been properly prepared. The employee sustained burns on the back of their legs from the molten aluminum that made contact with the sow mold. The employer did not ensure that the sow mold was adequately prepared to prevent the reaction of molten aluminum with the sow mold. Abatement note: Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow the practices outlined in AA GSC - Guidelines for Aluminum Sow Casting and Charging. |

Employees were required to be positioned in an associated danger zone that exists during machine operation in order to complete the tool change of the Cold Mill Machine. This condition exposed employees to nip points and crushed-by hazards. (b) Golden Aluminum, Inc. at 1405 E. 14th Street, Fort Lupton, CO 80621: On October 16, 2021, the employer did not ensure that alternative measures were developed, documented and utilized for the control of potentially hazardous energy when employees were required to engage in tool changes of the Cold Mill machine. An employee who was engaged in the tool change of the Cold Mill machine sustained a partial amputation of three fingers on his left hand. |

Abatement Note: Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: 1) A specific statement of the intended use of the procedures; 2) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; 3) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and 4) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures. |

Employees conducted hazardous operations while cleaning the floors and equipment of the paint line using Methyl ethyl ketone without gloves. Employees were exposed to toxic chemicals with known skin absorption hazards.
