29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) K.B.P. Coil Coaters Inc.: On or about January 23, 2024, and at times prior, employees were exposed to crushing and caught-in hazards while operating an unguarded Upender Machine. The Upender machine was not equipped with a guarding mechanism to prevent the platforms from coming down while employees were in the danger zone. b) K.B.P. Coil Coaters Inc.: On or about January 23, 2024, and at times prior, employees were exposed to caught in-hazards from rotating parts and struck-by hazards from flying chips while operating an unguarded JET Milling/Drilling Machine. c) K.B.P. Coil Coaters Inc.: On or about January 23, 2024, and at times prior, employees were exposed to laceration and amputation hazards while operating a JET 16" Horizontal Bandsaw that was not properly guarded. The adjustable guard on the band saw was not adjusted to minimize employee exposure to the unused section of the saw. Approximately four inches of the unused section of the blade was unguarded. |

Clearance space to Panel L3, Breaker 33, was impeded by a table placed directly in front of the electrical equipment. This condition exposed employees to the hazard of electrical shock and burns. b) K.B.P. Coil Coaters Inc.: On or about January 23, 2024, and at times prior, the employer did not ensure employees were protected from electrical and fire hazards in that, clearance around disconnect knife switch boxes were less than that specified in Table S-1. Clearance space to Panel LP, Breaker 21, was impeded by a pallet loaded with material placed directly in front of the electrical equipment. This condition exposed employees to the hazard of electrical shock and burns. |

This condition exposed employees to lacerations and finger amputations. b. K.B.P. Coil Coaters, Inc., 3600 E. 44th Ave, Denver, CO 80216: On and before May 11, 2022, the employer did not protect employees from pinch points while operating the slitter machine. It was observed the rollers were not guarded. This condition exposed employees to finger amputations. c. K.B.P. Coil Coaters, Inc., 3600 E. 44th Ave, Denver, CO 80216: On and before May 11, 2022, the employer did not ensure that the yellow barrier guards at the the slitter machine were closed allowing employee access in the machine area. The condition exposed employees to hazards associated with machines or equipment containing hazardous energy during set-up operations, change-up of cutting wheels, service and maintenance. 1. YODER slitter, 36-inch 2. Dahlstrom slitter, 48-inch 3. Hydraulic coil push-off ram 4. Rotating coil Mandrel re-winder Abatement Note: The training shall include the following: 1) Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. 2) Each affected employee shall be instructed in the purpose and use of the energy control procedure. 3) All other employees whose work operations are or may be in an area where energy control procedures may be utilized, shall be instructed about the procedure, and about the prohibition relating to attempts to restart or re-energize machines or equipment which are locked out or tagged out. |

Coil Coaters, Inc. at 3600 East 44th Avenue, Denver, CO 80216: On or before August 9, 2016, as part of injury/accident prevention responsibilities under its safety program, the employer did not ensure adequate machine guarding was provided to protect each operator and other employees from hazards such as those created by unprotected cutting wheels as part of point-of-operation of the YODER 36-inch slitter machine, in the slitting department, during the operating cycles. This condition exposed employees to amputation hazards when contacting the slitter's sharp cutting blades. (b) K.B.P. Coil Coaters, Inc. at 3600 East 44th Avenue, Denver, CO 80216: On or before August 9, 2016, as part of injury/accident prevention responsibilities under its safety program, the employer did not ensure adequate machine guarding was provided to protect each operator and other employees from hazards such as, but not limited to those created by point of operation, ingoing nip points, rotating parts, pinch points, flying chips and sparks during the operating cycles of the YODER 36-inch slitter, Dahlstrom slitter 48-inch, in the slitting department, not limited to the following instances. On August 9, 2016 a machine operator running the Dahlstrom slitter 48-inch was caught by the unguarded in-running nip points of the rotating coil re-winder resulted in a fatality. 1.
