29 CFR 1910.23(c)(1): Open sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides: (a)Munro Companies at 820 S. 9th St., Grand Junction, CO 81501: The mezzanine area in pump and manufacturing was not guarded by a standard guardrail in that there was no guard rail. This condition exposed the employee to a fall hazard of approximately 9.5 feet. Abatement Note: The following is provided to assist in the abatement of this hazard: (1)A standard railing shall consist of top rail, intermediate rail, and posts, and shall have a vertical height of 42 inches nominal from upper surface of top rail to floor, platform, runway, or ramp level. The top rail shall be smooth-surfaced throughout the length of the railing. The intermediate rail shall be approximately halfway between the top rail and the floor, platform, runway, or ramp. The ends of the rails shall not overhang the terminal posts except where such overhang does not constitute a projection hazard. (2)A standard toeboard shall be 4 inches nominal in vertical height from its top edge to the level of the floor, platform, runway, or ramp. It shall be securely fastened in place and with not more than 1/4-inch clearance above floor level. It may be made of any substantial material either solid or with openings not over 1 inch in greatest dimension. |

29 CFR 1910.179(j)(1): Cranes in regular service shall be inspected to insure compliance with the provisions of this section. The inspection intervals in turn are dependent upon the nature of the critical components of the crane and the degree of their exposure to wear, deterioration, or malfunction: (a)Munro Companies at 820 S. 9th St., Grand Junction, CO 81501: The employer did not ensure that all overhead cranes located throughout the Parts and Service had regular inspections. This condition exposed the employees to a struck-by hazard. Abatement Note: Inspection procedure for cranes in regular service is divided into two general classifications based upon the intervals at which inspection should be performed. The intervals in turn are dependent upon the nature of the critical components of the crane and the degree of their exposure to wear, deterioration, or malfunction. The two general classifications are herein designated as "frequent" and "periodic" with respective intervals between inspections is defined as: Frequent inspection - Daily to monthly intervals. Periodic inspection - 1 to 12-month intervals. |

29 CFR 1910.134(c)(1): A written respiratory protection program with worksite specific procedures, as specified in subparagraphs (c)(1)(i) through (ix) of this section, was not established and implemented where respirator(s) were required by the employer: (a)Munro Companies at 820 S. 9th St., Grand Junction, CO 81501: The employer did not have a written respiratory protection program with worksite specific procedures, as specified in subparagraphs (c)(1)(i) through (ix) of this section. The program was not established and implemented whenever respirators were required by the employer, in that the employer required the use of a half-face, negative pressure, air-purifying respirator during grinding operations in pump and manufacturing. This hazard exposed the employee to the lack of knowledge of proper respirator use. Abatement Note: The employer shall include in the program the following provisions of this section, as applicable: (1)Procedures for selecting respirators for use in the workplace; (2)Medical evaluations of employees required to use respirators; (3)Fit testing procedures for tight-fitting respirators; (4)Procedures for proper use of respirators in routine and reasonably foreseeable emergency situations; (5)Procedures and schedules for cleaning, disinfecting, storing, inspecting, repairing, discarding, and otherwise maintaining respirators; (6)Procedures to ensure adequate air quality, quantity, and flow of breathing air for atmosphere-supplying respirators; (7)Training of employees in the respiratory hazards to which they are potentially exposed during routine and emergency situations; (8)Training of employees in the proper use of respirators, including putting on and removing them, any limitations on their use, and their maintenance; and (9)Procedures for regularly evaluating the effectiveness of the program. |
