Murray & Stafford, Inc., at 14802 N. Grant Street, Thornton, CO 80023: On or before February 1, 2022, Murray & Stafford Inc., as the exposing and controlling employer, did not protect its employees and four employees from Rogers and Sons Inc., subcontractor from slips and fall hazards. The employer did not inspect and mitigate the ice on the walkway to the rooftop work area. One worker slipped on ice and was injured. This condition exposed employees to broken bones and head injuries. |

29 CFR 1926.451(g)(4)(xv): The employer did not install crossbracing to be acceptable in place of a midrail when the crossing point of two braces is between 20 inches (0.5 m) and 30 inches (0.8 m) above the work platform or as a toprail when the crossing point of two braces is between 38 inches (0.97 m) and 48 inches (1.3 m) above the work platform. The end points at each upright shall be no more than 48 inches (1.3 m) apart: a) On or before November 17, 2021, and at times prior, the controlling employer, Murray & Stafford, Inc., did not ensure that subcontracted employees were protected by guardrail systems while working on a supported scaffold of approximately 30 feet high. Cross braces were used as guardrail systems on the scaffold without a mid rail installed. The height of the cross brace crossing points were 48 inches high. This condition exposed employees to fall hazards of more than 10 feet. |

Murray & Stafford Inc directed the work of Powers Products Company at the Covidien facility. Employees of Powers Products Company applied Camger Coatings Systems Inc "Clear Additive Free 2K Acrylic" and "Clear Weatherseal Cure" to the Kalwall of Building 4 on 7/1/12. The MSDS's for Camger Coatings Systems Inc "Clear Additive Free 2K Acrylic" and "Clear Xylene QD Enamel Reducer" were incomplete. This condition potentially exposed an employee of Murray & Stafford Inc to a chemical hazard. |
