Citation text line 29 CFR 1910.176(b): Storage of material shall not create a hazard. Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse. (a) Target Corporation @ 715 Blue River Parkway, Silverthorne, CO. On or before April 10, 2013, the employer did not ensure that the pallets used on the metal racks at the receiving aisle were securely stacked in that, the upper wood pallets were sitting unevenly balanced and stacked extending over the edge of the 36"wide metal rack system and lacked any other suitable decking or shelving. Pallets used in this matter could potentially result in shifting or displacement. This condition exposed employees to strike-by hazards associated with material falling off, pallet sliding, and collapse from a 10 feet height.

An injury which occurred on December 15, 2010, and involved medical treatment and days away had not been recorded on the OSHA 300 injury and illness log.\\\\r\\\\n\\r\\n\\r\\nb) On or about May 18, 2011, and at times prior, the injury and illness log (OSHA Form 300) for 2010 did not contain entries for all injuries and illnesses which occurred in 2010. An injury which occurred on November 19, 2010, and involved one day away had not been recorded on the OSHA 300 injury and illness log.\r\n\r\n\r\nTarget Corporation was previously cited for a violation of Occupational Safety and Health Standard 29 CFR 1904.29(b)(1) or its equivalent which was contained in OSHA inspection 314623638, Citation 2, Item 1, issued August 19, 2010 with respect to a workplace located at 4255 Hampton Avenue, St. Louis, MO 63109. This citation became final by October 5, 2010. The final abatement date for this citation was October 5, 2010.\r\n\r\nTarget Corporation was previously cited for a violation of Occupational Safety and Health Standard 29 CFR 1904.29(b)(1) or its equivalent which was contained in OSHA inspection 311942858, Citation 1, Item 1a, issued May 8, 2008 with respect to a workplace located at 850 W. The OSHA 300A Summary Form indicated that there were 1 case with days away from work, 2 cases with job restriction or transfer and 1 other recordable case, however, the injury and illness log for 2009 (OSHA Form 300) showed that there were 2 cases with days away from work, 2 cases with job restriction or transfer and 1 other recordable case. \\r\\n\\r\\nc) On or about May 25, 2011, the OSHA 300A Summary Form provided to OSHA for calendar year 2010 was not complete and accurate. The OSHA 300A Summary Form indicated that there were 1 case with days away from work and 3 cases with job restriction or transfer, however, OSHA received two different injury and illness logs (OSHA Form 300) that had conflicting information. The OSHA Form 300 which was provided to OSHA on May 18, 2011, showed that there were no cases with days away from work and 3 cases with job restriction or transfer while the OSHA Form 300 which was provided to OSHA on May 25, 2011, showed that there were 2 cases with days away from work and 3 cases with job restriction or transfer. \\r\\n\r\n\r\n\r\nTarget Corporation was previously cited for a violation of Occupational Safety and Health Standard 29 CFR 1904.32(a)(1) or its equivalent which was contained in OSHA inspection 311942858, Citation 1, Item 1b, issued May 8, 2008 with respect to a workplace located at 850 W.
